To bypass the EULA acceptance screen if you get a 500 error:

Connect to SSPI and run 

sudo sqlite3 /config/db/operator_info.db "CREATE UNIQUE INDEX IF NOT EXISTS idx_operator_infos_name ON operator_infos (name);"
